Health Check is local service by Croydon NHS to enable you to gain advice on lifestyle changes to help maintain and improve your health if you are not already diagnosed with a long term condition and you are over 40 of age. Eligible patients will be invited by letter from Croydon NHS to visit the pharmacy of which Croydon Pharmacy is one several.
