Discharge support & reablement

If you or a family member has recently been discharged from hospital please let one of the pharmacy team know. If you have been issued a discharge letter by the hospital can you also please give this to the Pharmacist. This service has been introduced by NHS wales to help speed up the transition of information from the hospital to your GP.
